ACCIDENT WAIVER AND RELEASE OF LIABILITY
I hereby assume any and all risks of participating and/or volunteering with Save Maumee Grassroots Organization, Inc. (“SMGO”) during any event, meeting, activity, and/or volunteer opportunity sponsored and/or associated with SMGO. I realize that accidents and/or illness and/or bodily injury and/or damage to personal property may arise from negligence or carelessness on the part of person(s) and/or because of dangerous terrain, including, but not limited to, pollution and wildlife. I specifically release SMGO and its affiliates, directors, officers, employees, volunteers, representatives, and agents from any and all liability, claims, demands, obligations, actions, causes of action, damages, costs, attorney fees, expenses, and compensation of any nature whatsoever arising out of my participation and/or involvement with any event, meeting, activity, and/or volunteer opportunity sponsored and/or associated with SMGO, now and in the future.
By signing this Accident Waiver and Release of Liability, I agree to indemnify and hold harmless SMGO and its affiliates, directors, officers, employees, volunteers, representatives, and agents from any and all liabilities, loss, claims or damage whatsoever which may result from my participation in any event, meeting, activity, and/or volunteer opportunity sponsored and/or associated with SMGO, whether caused by negligence of any person, dangerous terrain, or otherwise, now and in the future.
I hereby consent to receive medical treatment that may be deemed advisable in the event of injury, accident, and/or illness that may be caused from participation in any event, meeting, activity, and/or volunteer opportunity sponsored and/or associated with SMGO.
I understand that during any event, meeting, activity, and/or volunteer opportunity sponsored and/or associated with SMGO, I may be photographed or videotaped. I agree to allow my photo or likeness, video or film, with or without my name, to be used for any legitimate purpose by the event holders, producers, sponsors, organizers and assigns of SMGO.
The Accident Waiver and Release of Liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law. Please do not participate in any event, meeting, activity, and/or volunteer opportunity sponsored and/or associated with SMGO if you decline signatures.
